The bristles of this brush are very soft, but they became frayed within a week, which has never happened with any other brush, including cheaper ones. Great brush - dense bristles that don't scratch the gums. It effectively removes plaque buildup. As for the firmness, in my opinion, it falls somewhere between medium and soft. It's a shame that there isn't a tongue cleaning pad included.
